Custom Shopify registration form with Cosmo

Cosmo replaces Shopify's default registration form with a sign-up form you design. Add your own questions and file uploads, a phone number with country code and an email marketing consent checkbox, and every answer is saved to the customer in Shopify the moment they sign up.

How it works

How does Cosmo's custom registration form work?

Switch the form on and choose its fields in Cosmo's editor in Shopify admin. Below is what a customer fills in, where it is saved and how the form looks.

The form

What can you add to the Shopify sign-up form?

Cosmo's sign-up form always asks for first name, last name and email, the three details Shopify needs to create a customer. On top of those you add your own questions as text, choice, date or file upload fields, mark each one required or optional, and set how wide it sits on the form.

On the form

  • Text and multi-line text answers
  • Single-choice and multiple-choice questions
  • Date fields, such as a birthday
  • File uploads: images, PDFs or Word documents
  • Required or optional, field by field
  • An email marketing consent checkbox

Phone number

Can customers sign up with a phone number and country code?

Yes, once you switch the phone field on. Customers pick their country from a flag and dial-code list, type their local number, and Cosmo saves it on the Shopify customer with the country code. You decide whether the phone number is optional or required to sign up.

  • Off until you switch it on
  • Optional or required, your choice
  • Flag and dial-code country picker
  • Country suggested from the visitor's location
  • Saved to Shopify with the country code

Your data

Where are sign-up answers saved in Shopify?

On the customer record in Shopify, created the moment the form is sent. Name, email and phone fill Shopify's own fields, and the answers to your questions are saved as customer metafields in the same step, so the data stays in Shopify for segments and marketing. Uploaded files are stored in Shopify too.

  • A Shopify customer, created when the form is sent
  • Custom answers saved as customer metafields
  • Uploaded files stored in Shopify, linked to the customer
  • Email consent recorded only when ticked
  • Existing emails asked to sign in, not duplicated

Design

Can the sign-up form match your store's design?

Yes. In Cosmo's editor in Shopify admin you set the form's text sizes, its width and alignment, one or two columns of fields, and the colours of the page, form, headings and labels. You can add an image to the left or right of the form, and a preview shows each change.

  • Heading, sub-heading and body text sizes
  • Form width and alignment
  • Full-width or two-column fields
  • Page, form, heading and label colours
  • An image left or right of the form

Two-column fields show on tablets and wider; phones get one column

Questions about Shopify sign-up forms

Anything else? Email info@cosmoinfotech.com and it goes straight to the developer.

Can I add custom fields to the Shopify registration form?

Yes. With Cosmo you add your own questions to the sign-up form as text, multi-line text, single-choice, multiple-choice, date or file upload fields, and mark each one required or optional. The answers are saved to the customer in Shopify as customer metafields, and the customer can update them later on their profile page.

Can customers upload files when they sign up?

Yes. Add a file upload field to the Cosmo sign-up form and customers attach a file as they register, such as a photo, an ID scan or a certificate. You choose whether each field accepts images (JPG, PNG, WebP or GIF), PDFs or Word documents, up to 10 MB a file. The file uploads straight to Shopify and is saved to the customer's record.

Where does the sign-up form appear on my store?

On the account page. When the sign-up form is switched on in Cosmo, a signed-out visitor who opens the account page sees your form there instead of being sent straight to Shopify's login page. While it is switched off, visitors go to Shopify's login page as before.

Is a phone number required to sign up?

Only if you make it required. The phone field is off until you switch it on in Cosmo, and then you choose whether customers must fill it in. Customers pick their country from a dial-code list, so the number is saved in Shopify with its country code.

Can the sign-up form collect email marketing consent?

Yes. Cosmo shows an email marketing consent checkbox on the sign-up form, which you can hide or reword. When a customer ticks it, they are subscribed to email marketing on their Shopify customer record as a single opt-in. When they leave it unticked, no consent is recorded and they are not subscribed.

What happens if someone signs up with an email that already has an account?

Cosmo checks for an exact match on the email before creating anything. If the address already belongs to a customer, the form tells them they already have an account and offers a sign-in button, so no duplicate customer is created in Shopify.

Do I need to edit my theme code?

No. Setup runs through a 4-step guided flow inside Shopify admin, and the customer account page is published from there. The loyalty program's product-page rewards widget is added as a block in the theme editor. No developer is required and no theme files are edited.
